OpenID server_error: An unexpected error occurred.

Description: 
When attempting to use an OpenID realm, it fails with server_error: An unexpected error occurred.

Cause:
This can happen when the scope is missing from the request.
 
Resolution: 

1. Grab the full address from the address bar, including the query string. It will look something like:

"https://Secureauth.example.com/SecureAuth123/Authorized/OidcAuthorize.aspx?

response_type=code&client_id=123f65a1232212389c952123b56cea16

&redirect_uri=https%3a%2f%2fexample.oidc.com&state=A2SAAEAEMFNg7l"

2. Check that Scope is in the query string (it's missing in the above example)

3. Edit your OIDC server so it requests the scope.
